  • Passports issued to applicants age 15 & under are valid for 5 years
  • Passports issued to applicants age 15 & under are not renewed, applicants must follow new passport instructions
  1. Include one of the following:
    • Old Passport (undamaged), OR
    • Official government issued Birth Certificate (if born in the US), OR
    • Certificate of citizenship (if born abroad to US parent(s), OR
    • Naturalization papers (for US citizens through naturalization)
  2. Proof of citizenship MUST be an original or certified copy. No photocopies or notarized copies are accepted
  3. The original document will be return to you with your completed passport
  1. Include one of the following:
    • Official Government issued Birth Certificate listing Parent(s), OR
    • Adoption Decree (with adopting Parent(s) names, OR
    • Court order establishing guardianship, OR
    • Certificate of citizenship listing parent(s), OR
    • Naturalization papers listing parent(s)
  2. The document MUST be original or certified copy. NO photocopy or notarized copies are accepted
  3. The original document will be return to you with your passport
  4. Please not that this may be the same document(s) that proves the child’s citizenship. In other words if you are using a birth certificate for proof of citizenship, the same document might prove parental relationship. There is no need to include two in this case
The Department of State requires that BOTH parents consign to the issuance of a passport for child under age of 15 one of the following options must be used:
  • Both parents appear together in front of the Passport Acceptance Agent (see authorization below on step 2) to sign the government application form DS.11, OR
  • One parent appears and submits primary evidence of sole authority to apply, OR
  • One parent appears and submits the second parent’s written statement (DS.3053)of consent authorizing the Department of State to issue the passport
  • The DS.3053 can be downloaded HERE or picked up at a Passport Acceptance Agent facility
  • The DS.3053 MUST be notarized
  • A photocopy of the parent’s valid driver’s license. Issued more than 6 months ago
  • If the applicant does not possess a valid driver’s license (state ID / Military ID), or if the license was issued LESS than 6 months ago (even renewals) CLICK HERE for additional proof of identity instructions
Get application authorized (applicant identity verified and ds.11 signature witnessed).
All first time applicants need to have their applications authorized by local government officials called Passport Acceptance Agents.
  1. Passport Acceptance Agents are located in Post Offices and Courthouses around the country. Not every Post Office or Courthouse has an Acceptance Agent, and hours are vary, CLICK HERE to find one in your area
  2. The Passport Acceptance Agent job is to witness the applicant sign the government for DS.11, check their ID and seal some of the application materials into an envelope
  3. The items that are sealed are:
    • The signed DS.11 application form
    • The original proof of citizenship
    • One passport type photograph
    • The check or money order to US Department of State $170
  4. The items that are not sealed:
    • The second copy of the DS.11 application form
    • The 2 copies of letter of authorization
    • The photocopy of the applicant’s driver’s license or the supplemental ID
    • The proof of departure (required for same day or next day service only)
    • One copy of the completed PDF order form
Tell the Passport Acceptance Agents that you are using Visa Obtainers, Inc. to expedite your passport application. They must return the sealed envelope to you
  1. Passport Card
    • The new passport cared cannot be expedited at this time and take about 4-6 weeks to issue, there is additional $30 government fee for the passport card
    • The passport card will be processed separately from your passport
    • The passport card will be mailed directly to you
  2. Child Letter of Authorization
    • CLICK HERE for child letter of authorization, print TWO copies
    • Letter of Authorization MUST be signed by the parent(s) who co-sign the application form DS.11
    • Letter of authorization must have original signature, fax, scans or photocopies are not acceptable
  3. Passport photograph:
    • One passport type photograph is required
    • Photo must be recent, taken within last six months, 2x2 inches, front view, full face, in color and on plain light/white background. Digital and non-passport type photographs are not acceptable CLICK HERE for more on photos
  4. Proof Of Departure:
    • Copy of travel itinerary from airline or travel agency with applicant name, travel date and flight information, OR
    • Copy of airline ticket showing foreign departure, OR
    • If travel for business and neither of the above is available, CLICK HERE for sample business letter
    • Proof of travel is required for next day service ONLY
  5. Government Fee(s):
    • $140 check or money order payable to US department of State (write your name and date of birth on the check or money order)
    • $25 collected buy your local Passport Acceptance Agent (explained below)
The Passport Agency process a limited number of passport applications in the next day. If your international departure requires next day service, call us immediately at (202) 347-2909 or by email at info@visaobtainers.com to reserve a same day slot.

You must complete the Visa Obtainers PDF Order Form

The order form gives us your name, address, shipping information and payment method. Multiple applicants can be entered using one order form. You only need to complete one order form per request. While completing the online order form in this step, you will be asked how you want to pay the processing fees. If you are paying by credit card, please complete the credit card information on the online or PDF order form. You may pay by business, personal check or money order payable to Visa Obtainers, Inc.

Shipping and Special Instructions

IMPORTANT:
  1. If your international departure requires next day service, you must ship your passport materials via Federal Express priority overnight for delivery by 10:30AM ONLY, CLICK HERE to search for FedEx location.
  2. If your international departure is not requires next day service ship your passport materials at next day delivery speed using expedited service, FedEx, UPS, ExpressMail, etc.
  3. After seeing the Passport Acceptance Agent call (202) 347-2909 with the following ready:
    • The government sealed envelope
    • The photocopy of applicant’s driver’s license or the supplemental ID
    • The TWO copies of letter of authorization
    • The proof of departure (required for next day service only)
    • One copy of the PDF online order form
  4. We will checklist your passport materials over the phone and you can make us aware of any special circumstances
  5. We hand carry your passport application to the US Passport Agency
  6. We will return your new passport via secure traceable overnight service you select
  7. Non-refundable tickets should not be purchased until all your visas and passports are secured and in your possession
