1. This process is for replacing a lost or stolen passport when:
   • The applicant is currently age 16 & over AND
   • The missing passport is still valid (i.e. not expired)
2. If the missing passport has expired, click for first time passport instructions
3. Applicants currently age 15 or under, click for child passport instructions
4. Applicants with an expired / expiring passport: if the passport still in your possession, AND was issued less than 15 years ago, AND you were age 16 or over when was issued, click for passport renewal instructions

1. Include one of the following:
   • Old Passport (undamaged), OR
   • Official government issued Birth Certificate (if born in the US), OR
   • Certificate of citizenship (if born abroad to US parent(s)), OR
   • Naturalization papers (for US citizens through naturalization)
2. Proof of citizenship MUST be an original or certified copy. NO photocopies or notarized copies are accepted.
3. The original document will be return to you with your completed passport.

• Letter of authorization must have original signature, fax, scans or photocopies are not acceptable

• A photocopy of the applicant’s valid driver’s license. Issued more than 6 months ago
• If the applicant does not possess a valid driver’s license (state ID / Military ID), or if the license was issued LESS than 6 months ago (even renewals) CLICK HERE for additional proof of identity instructions.
• Applicants age 16 or 17 without a valid driver’s license can have a parent co-sign the application.In this case a Birth Certificate proving parental relationship is required for the application.

• The new passport cared cannot be expedited at this time and take about 4-6 weeks to issue, there is additional $30 government fee for the passport card
• The passport card will be processed separately from your passport
• The passport card will be mailed directly to you

• One passport type photograph is required
• Photo must be recent, taken within last six months, 2x2 inches, front view, full face, in color and on plain light/white background. Digital and non-passport type photographs are not acceptable CLICK HERE for more on photos

• Copy of travel itinerary from airline or travel agency with applicant name, travel date and flight information, OR
• Copy of airline ticket showing foreign departure, OR
• If travel for business and neither of the above is available, CLICK HERE for sample business letter
• Proof of travel is required for same day or next day service ONLY

Two separate checks or money orders:

• $170 check or money order payable to US department of State (write your name and date of birth on the check or money order)
• $25 collected buy your local Passport Acceptance Agent (explained below)

Get application authorized (applicant identity verified and DS.11 signature witnessed)

All first time applicants need to have their applications authorized by local government officials called Passport Acceptance Agents. Bring all the materials listed above. This section describes what happens there.

1. Passport Acceptance Agents are located in Post Offices and Courthouses around the country. Not every Post Office or Courthouse has an Acceptance Agent, and hours are vary, CLICK HERE to find one in your area
2. The Passport Acceptance Agent job is to witness the applicant sign the government for DS.11, check their ID and seal some of the application materials into an envelope.
3. The items that are sealed are:
   • The signed DS.11 application form
   • The DS.64 form completed and signed
   • The original proof of citizenship
   • One passport type photograph
   • The check or money order to US Department of State $170
4. The items that are not sealed:
   • The second copy of the DS.11 application form
   • The 2 copies of letter of authorization
   • The photocopy of the applicant’s driver’s license or the supplemental ID
   • The proof of departure
   • One copy of the completed PDF online order form
5. Tell the Passport Acceptance Agents that you are using Visa Obtainers, Inc. to expedite your passport application. They must return the sealed envelope to you. It is occasionally necessary to refer the Passport Acceptance Agent to the US Department of State’s “Hand Carrying of Executed Applications” policy.
