The Passport Agency process a limited number of passport applications in the same or next day. If your international departure requires same day service, call us immediately at (202) 347-2909 to reserve a same or next day slot.
Select this process to have a passport amended to reflect name change.
Note: Passports MUST reflect the legal name in order to travel internationally. You should use this process if your passport is LESS than ONE year old AND you can supply official document(s) indicating the change name.
If the passport is more than year old , you must renew the passport CLICK HERE for passport renewal instructions. You cannot have the name change on a passport if you cannot supply official documentation showing the name change

- Applicants must submit the official document(s) indicating name change
- These include marriage certificate, divorce decree (specifically stating name change), or court paper
- The Passport Agency does not accept photocopies of any of these forms, official documents will be return with your new passport

- Letter of authorization must have original signature, fax, scans or photocopies are not acceptable
- One passport type photograph is required
- Photo must be recent, taken within last six months, 2x2 inches, front view, full face, incolor and on plain light/white background. Digital and non-passport type photographs arenot acceptable CLICK HERE for more on photos
$60 check or money order payable to US Department of State. Please write your name and date of birth on the check or money order.
You must complete the Visa Obtainers PDF Order Form
The order form gives us your name, address, shipping information and payment method. Multiple applicants can be entered using one order form. You only need to complete one order form per request. While completing the online order form in this step, you will be asked how you want to pay the processing fees. If you are paying by credit card, please complete the credit card information on the online or PDF order form. You may pay by business, personal check or money order payable to Visa Obtainers, Inc.

- If your international departure requires next day service, you must ship your passport materials via Federal Express priority overnight for delivery by 10:30am only, CLICK HERE to search for FedEx location
- If your international departure is not requires same day or next day service ship your passport materials at next day delivery speed using expedited service, FedEx, UPS,Express Mail, etc.

- Valid passport
- Official name change documentation
- Passport application form DS.5504 completed online and signed
- One passport type photograph
- Two Copies of letter of authorization
- Proof of departure (required for same day or next day service only)
- Copy of the online completed PDF order form
- We will checklist your passport materials over the phone and you can make us aware of any special circumstances.
- Be sure to send the passport application materials as listed on step one, any discrepancy can cause a delay
- We will return your new passport via secure traceable overnight service you select
